If you have a Medicaid plan not listed above (United Healthcare Plan for example), please call 1-800-562-3022 to switch plans. Molina and CHPW are very easy to work with. This process should
take one phone call.
We accept most private insurance companies or can bill out-of-network if needed.
We highly recommend that prior to scheduling your meet and greet, you either call your insurance company and ask them to verify your benefits in writing for you, or you can complete the form below and we will do it for you:
