An Estimate of Benefits (EOB) is REQUIRED prior to your Initial Visit.

 

This helps you understand what your total financial obligation will be and we will make a payment plan with you based on the EOB.

 

You can do this by yourself by calling your insurance company or our biller can do it for you for a $20 fee.  

 

To have our biller complete this process:

• Select the PATIENT SERVICES tab, then select VERIFICATION OF BENEFITS 

• Scroll down and select REGISTER HERE 

• Complete and submit the Patient Registration Form
